What Do We Need To Ask A Psychiatrist For A Formal Diagnosis Of Depression/anxiety?

For the first time in my lifetime, I'm seeing a psychiatrist instead of a psychologist this week for a formal diagnosis of depression and anxiety (I'm already on 20mg of Paroxetine, which isn't entirely helpful), as well as possible (probable) bipolar.
