What Do We Need To Ask A Psychiatrist For A Formal Diagnosis Of Depression/anxiety?

For the first time in my lifetime, I'm seeing a psychiatrist instead of a psychologist this week for a formal diagnosis of depression and anxiety (I'm already on 20mg of Paroxetine, which isn't entirely helpful), as well as possible (probable) bipolar. To help jump-start the process, what do I need to ask and/or tell the psychiatrist? Thank you, friends!!

Can I jump in here?
My advice to anyone going for an evaluation with a medical health professional, take an hour or so before hand and write down everything that makes you feel the way you do.
Don’t miss anything important or even things you may think are not so important out of your list. There’s nothing worse than leaving the doctors office, thinking about the things you forgot to mention.
This can happen because you have so much going on in your head whilst sitting in front of the doctor.
It’s always beneficial to have a reference note to tick off things discussed along the way. Best of luck and be strong 🙂

Thank you! My appointment is Tuesday, and the two psychologists I've seen gave me diagnoses but not in easily-understood terms (years ago when we were "manic depressive" and "bipolar" was not a term yet). Thanks again!

Thank you for excellent answers, friends!! My appointment on Tuesday was part one of two, and it was incredibly successful, as I did indeed had all my list of questions prepared—either written down in my mind or in a jot sheet with my medication that I didn't need to pull out. My part two of two appointment is May 16 when I hope to solidify two diagnoses (depression/anxiety and bipolar), which were never properly diagnosed by a psychologist. My therapist is top-notch and left me with *all* questions asked for this session—a sigh of relief. So far, so good! Thank you, everyone, for your support, thoughts and prayers, which means a LOT!!!

I went through this in March. The place had forms that I filled out online. The session was for a hour and a half. The only thing that I told the doctor was I have been dealing with this for five years and the antidepressants stopped working and my doctor told me their are no more antidepressants left to try. I told that I have MDD she sajested katamine and TMS therapy.
